While it is in your hotbar it passively negates the first attack Michael lands on you, blinding him for about three seconds. It does not need to be activated. Like other items it can also be thrown for a stun.
The Ancient Amulet is the one rare item in Halloween: The Game whose effect is actually settled. It is a passive: as long as it sits in your hotbar, the first attack Michael lands on you is negated and he is blinded for around three seconds. You do not activate it and there is no prompt — it simply spends itself the first time it matters.
That makes it worth more in the situations where a single grab ends a run rather than costs you a few seconds. Carrying an escape objective is the obvious one: without the amulet a grab can take both you and the item out of your team's hands, and with it the grab becomes a window instead. The same applies when you are the one going in to rescue a teammate — the blind buys the rest of the group time to act.
It is rare enough that finding one is not something you plan a match around. Treat it as a windfall that changes what you are willing to risk for the rest of the round, not as a build.
